Make Your Wishes Known Initiative

The Initiative is a collaboration of the Coalition, the Hospice Caring Project, and the Health Improvement Partnership of Santa Cruz County. The goal of the project is to increase community awareness of the importance of advance care planning and completing an Advance Directive for Health Care.  

In the Fall of 2006, Initiative volunteers will distribute thousands of information cards to most doctors’ offices in the County, urging patients to fill out an Advance Directive. The Initiative sponsors public informational events and provides speakers and counselors for individuals, families and community groups wishing help in completing their directives.
